Instructions For completing the form:
All students must fill in the top of Page One, (“Application Information”): (Your Social Security No., Last Name, First Name and Middle initial) and the top of Page Two: “Applicant Social Security Number”.

Next: Review the rest of the form and make changes
only in the boxes where information needs to be added or changed.

Last: “EVERYONE WHOSE INCOME INFORMATION IS INCLUDED ON YOUR APPLICATION MUST SIGN AND DATE THE AFFIRMATION BOX” (bottom of page Two).
